According to the Travel Information
Manual (TIM), the US permits a passport bearer to include
his/her unmarried minor children, under the age of 13, in his/her
passport (Nov. 1994, 362). The children cannot, however, use the
passport to travel unless accompanied by the passport bearer
(ibid.). An individual application form (Non-Immigrant Visa
Application Form OF-156) must be submitted for each person,
irrespective of age, wishing to travel to the US. This requirement
also applies to children included in the parent's passport (ibid.,
