Information on certificates of reports filed in Haiti was scarce among the sources consulted by the Research Directorate within the time constraints of this Response.

In a telephone interview with the Research Directorate, an official at the Departmental Service of Judicial Police (Service départemental de la police judiciaire, SDPJ) of the Centre department indicated that when a report is filed, a form is completed by the police officer (Haiti 1 Nov. 2018). In correspondence with the Research Directorate, the SDPJ official provided the Research Directorate with two sample forms for filed reports that were used until September 2018 (Attachments 1 and 2) and two samples for filed reports that have been used since September 2018 (Attachments 3 and 4); however, no details were provided as to the differences in usage.

According to information provided by the SDPJ official, reports are recorded in the [translation] “Report Book” and the individual who filed the report may obtain a copy of the form by making a request in person or through a lawyer (Haiti 1 Nov. 2018). The copy is signed by the head of investigations of SDPJ and is stamped (Haiti 1 Nov. 2018). Corroborating information could not be found among the sources consulted by the Research Directorate within the time constraints of this Response.

In correspondence with the Research Directorate, the SDPJ official added, without providing further details, that the police also issue certificates of filed reports to the individual who filed the report [translation] “depending on the length of time since the report was filed” (Haiti 2 Nov. 2018). Information on the process for obtaining a copy of the certificate of filed reports could not be found among the sources consulted by the Research Directorate within the time constraints of this Response.

Information on the appearance of the certificate of filed reports could not be found among the sources consulted by the Research Directorate within the time constraints of this Response, nor could information be found on whether there is a template certificate for all police stations in Haiti or a template certificate for filed reports for the police station in Hinche.
